PRE-K AND KINDERGARTEN DOCUMENTS NEEDED

Live within Nacogdoches ISD boundaries

The child’s birth certificate or one of the following:

hospital birth record, passport, military ID, adoption records, church baptismal record, or any other legal document establishing identity and age

Child’s immunization record

Photocopy of parent/guardian’s ID

Proof of residency, which can be one of the following:

Rental agreement, A water, gas, or electric bill, or a Landlord affidavit

ADDITIONAL REQUIREMENTS FOR PRE-K

Meet at least one of the following criteria:

Unable to speak and comprehend the English language

Qualifies for free or reduced lunch

Qualifies as homeless as defined by federal law

Currently in or has ever been in foster care

Is a child of an active duty member of the U.S. Armed Forces or the child of a member of the U.S. Armed Forces who was injured or killed while serving on active duty

Proof of Income

Meet at least one of the following criteria:

Current paycheck stub

Letter from employer stating gross wages paid and how often they are paid

Unemployment, worker’s compensation, or disability payment check stub

Previous year’s tax return

SNAP or TANF document card that contains the name of the child, current case number, and an adult signature

LAS Language Testing

If your child will be attending Pre-K or Kindergarten, is new to Nacogdoches ISD and speaks a language other than English, please call Mrs. Maria Marquez at 936-569-5000 ext. 8859 to make an appointment for the LAS Language Testing. This test is particularly important for those who wish to qualify for Pre-K. A few things to keep in mind:

· Must have completed the online registration prior to making an appointment.

· Test will be administered by appointment only.

· Language testing is a registration requirement for Pre-K & KG students who speak a language other than English.

District staff will review the submitted application to determine eligibility for free PreK and will notify a parent of the status of their application.
